About Your Visit
We are pleased to welcome you to Pediatric Cardiology Associates. We are providing this information so that we may better serve you. Here you will find answers to the questions most patients ask. However, if you have other questions, please feel free to call our office.
Appointments
We see all patients on an appointment basis, and ask that you call in advance so that we may reserve time for you. The office phone number is 207-773-2723. The office is open Monday through Friday from 9:00 a.m. to 5:00 p.m. If an emergency arises while the office is closed, please call our office at the number above and our 24 hour answering service will promptly forward your message to the physician on call. You will receive a response as quickly as possible. Each of our physicians has access to your health records.
Fees, Payments & Insurance
We make every effort to keep down the cost of your medical care. Health care insurance is intended to cover some, but not all, of the cost of your treatment. Most plans include co-insurance, a deductible and other expenses which must be paid by the patient.
If you have insurance, please bring your plan information with you. We will work with you to assure that you receive the maximum benefits to which you are entitled. Our office submits your charges directly to your insurance company for you. If you have questions regarding your coverage, please ask a member of our office staff and we will work with you to find the answer.
We are participating providers with many health maintenance organizations (HMO's) and preferred provider organizations, including Maine and New Hampshire Medicaid.
If you do not have any health insurance coverage, please contact our office staff. Programs may be available for which you may qualify to help with the cost of your medical care. Our first concern is for the health of our patients. If financial difficulties exist, it is important to let us know so that we can make mutually suitable arrangements.
Confidentiality
We place a high priority on maintaining confidentiality of medical records and information about your health care. We will not release any information without your signed authorization.

Important Insurance Information
If your insurance company requires a referral, make sure your primary care
doctor has called your insurance company to give you a referral number for
your appointment(s). It is very important that the referral is made either
to one of our doctors or Pediatric Cardiology Associates.
If you do not have a referral, you will be asked to sign a waiver that you are financially responsible for the appointment.
Please bring the following information with you to your visit:
1. Your insurance card.
2. The social security number of the person whose insurance will be billed for this visit.
3. The date of birth of the person whose insurance will be billed for this visit.
